Testosterone Replacement Therapy (TRT)

Testosterone replacement therapy restores testosterone to a healthy physiological range in men whose own production has genuinely fallen too low. At AmpUp Wellness, every TRT programme is built on two early-morning lab panels, prescribed and supervised by David Patterson, APRN, and adjusted in person over time — not issued from an online questionnaire.

Who Testosterone Replacement Therapy Is For

Testosterone declines slowly in most men from around age 30, and for many that decline never becomes a clinical problem. TRT is for the men in whom it does — where the numbers are genuinely low and the symptoms are real and persistent.

Diagnosis is never made on symptoms alone. Following American Urological Association guidance, we look for a total testosterone below roughly 300 ng/dL on two separate early-morning blood draws, paired with clinical symptoms, before we consider starting therapy.

Signs Worth Measuring

Any one of these has many possible causes. Several together, in a man over 35, is worth a blood test.

Persistent fatigue

Tiredness that a full night's sleep does not resolve, and that has not shifted for months.

Strength and muscle loss

Muscle mass slipping and lifts going backwards despite training that has not changed.

Weight around the midsection

Fat gathering centrally and resisting the diet and exercise that used to work.

Low libido or erectile quality

A clear drop from your own baseline, rather than an occasional off week.

Flat mood and motivation

Low drive, irritability and a sense of being blunted rather than actively depressed.

Brain fog and poor sleep

Difficulty concentrating, and sleep that is broken or no longer restorative.

And Who It Is Not For

We do not start testosterone in men who are actively trying to conceive, because exogenous testosterone suppresses the body's own sperm production. We also hold off in men with an untreated elevated haematocrit, untreated obstructive sleep apnoea, or a cardiovascular event within the last three to six months.

It Starts With Bloodwork, Drawn in the Morning

We measure total and free testosterone, SHBG, and oestradiol — because testosterone converts to oestrogen and that conversion needs watching. We add LH to distinguish a testicular cause from a pituitary one, a complete blood count for your baseline haematocrit, a metabolic panel, and PSA for men over 40.

Those results tell us three things: whether you are genuinely deficient, why, and whether it is safe to treat you.

Nobody gets a standard dose. We start conservatively, aiming for a mid-normal physiological range rather than a supraphysiological one, then recheck and adjust. Most men reach a stable dose within three to four months.

Injections or Pellets

AmpUp offers both. They suit different people, and most patients establish the right dose on injections first.

Injections

  • Cadence: weekly or twice-weekly
  • Where: self-administered at home after we teach the technique
  • Levels: comparatively steady — avoids the peak-and-crash of older three-weekly protocols
  • Adjustment: dose can be changed week to week, so it is the easiest method to fine-tune

Pellets

  • Cadence: one in-office procedure every three to five months
  • Where: implanted under the skin of the hip
  • Levels: gradual, sustained release
  • Adjustment: fixed once implanted — the dose cannot change until it wears off

What Changes, and When

Testosterone does not work overnight, and any clinic that tells you otherwise is selling something. Here is the realistic sequence, based on what our patients report and what the evidence supports.

The first six weeks

  • Weeks 1–2: sleep quality and morning energy are usually the first things to shift. Some men notice mood and mental clarity lifting in this window too.
  • Weeks 3–4: libido and sexual function typically begin to improve. Motivation and drive often follow.
  • Weeks 5–6: first follow-up labs. Energy gains tend to consolidate rather than keep climbing, and we confirm your level has landed where we intended.

Three to six months and beyond

  • Month 3: body composition starts to move — lean mass up, fat mass down — assuming training and nutrition are in place.
  • Month 6: strength gains, insulin sensitivity and bone-density benefits are established. Most men are on a stable dose by now.
  • Beyond: benefits are maintained, not compounded. TRT is ongoing therapy, and stopping returns you to baseline over several months.

Because the month-three changes are compositional rather than scale-weight changes, we track them with an InBody body composition scan rather than a bathroom scale. Watching lean mass rise while fat mass falls is the clearest evidence a protocol is working — and it is something a telehealth prescription cannot give you.

Monitoring Is the Part Online Providers Skip

Testosterone replacement changes your blood chemistry, and it deserves proper oversight.

Follow-up bloodwork. We recheck at six to eight weeks after starting or changing a dose, again at three months, then every six to twelve months once stable — consistent with American Urological Association guidance.

Haematocrit. The most common side effect is erythrocytosis, a dose-related thickening of the blood. It is manageable by lowering the dose, changing injection frequency, or donating blood — and it is entirely invisible without a blood test.

Prostate. For men 55 to 69 we assess risk before starting and again three to twelve months in, per Endocrine Society guidance. A PSA rise above 1.4 ng/mL over baseline in year one, or any result above 4.0 ng/mL, prompts a urology referral.

Fertility and cardiovascular risk. Testosterone suppresses sperm production, and for some men that does not fully reverse — tell us at consultation if fertility matters. On cardiovascular risk, the evidence remains genuinely inconclusive, and we will not overstate the benefit in either direction.

What Testosterone Replacement Therapy Costs

TRT is not a single-price product, because the labs, the delivery method and the dose all vary by patient. Rather than publish a headline figure that turns out not to apply to you, we price transparently at the point where we actually know what you need.

Your programme starts with a consultation and a baseline hormone and safety panel. Once those results are in, APRN David Patterson will walk you through the recommended protocol and the full monthly cost — medication, supplies and follow-up labs included — before you commit to anything. There is no obligation to proceed after the consultation, and we will tell you if we do not think TRT is the right answer for your symptoms.

Two things worth knowing up front. First, pricing differs between injections and pellets: injections carry a lower recurring cost, pellets carry a higher per-procedure cost spread across three to five months. Second, ongoing monitoring bloodwork is part of the therapy, not an optional extra — any quote that omits it is not a complete quote.

What is testosterone replacement therapy?
Testosterone replacement therapy, or TRT, is a prescribed medical treatment that restores testosterone to a healthy physiological range in men whose own production has fallen too low. It is delivered by injection or subcutaneous pellet, dosed to your lab results, and monitored with ongoing bloodwork. It is a long-term therapy rather than a course of treatment — benefits are maintained while you remain on it.
What are the signs of low testosterone?
The most common signs are persistent fatigue that rest does not resolve, loss of muscle mass and strength, increased body fat around the midsection, low libido or reduced erectile quality, poor sleep, low mood, irritability and difficulty concentrating. These symptoms overlap with thyroid problems, sleep apnoea, depression and simple overtraining, which is why diagnosis requires bloodwork rather than a symptom checklist.
How is testosterone therapy administered?
AmpUp Wellness offers testosterone by intramuscular or subcutaneous injection, typically weekly or twice-weekly and usually self-administered at home after we teach the technique, and by subcutaneous pellet implanted in-office and releasing gradually over three to five months. Injections allow the dose to be adjusted at any point; pellets trade that flexibility for convenience. Most patients establish their dose on injections first.
How long before I feel a difference on TRT?
Sleep quality and energy usually shift within the first two weeks. Libido and motivation typically improve between weeks three and four. Body composition changes — lean mass up, fat mass down — become measurable around month three, and strength and metabolic benefits consolidate by month six. Anyone promising dramatic results in the first fortnight is overselling it.
What labs are required before starting TRT?
Two early-morning total testosterone measurements on separate days, plus free testosterone, SHBG, oestradiol, LH, a complete blood count to establish baseline haematocrit, a comprehensive metabolic panel, and PSA for men over 40. Prolactin is added when LH returns low or low-normal. A single testosterone reading, particularly one drawn in the afternoon, is not sufficient to diagnose deficiency.
Is testosterone replacement therapy safe long-term?
For appropriately selected and properly monitored patients, TRT has a well-established long-term safety profile. The main risk requiring active management is erythrocytosis, a dose-related thickening of the blood detected through routine haematocrit testing and managed by dose adjustment or blood donation. Prostate monitoring continues throughout therapy. The evidence on cardiovascular risk remains inconclusive in both directions, and we counsel patients honestly about that uncertainty rather than claiming a benefit the data does not support.
Will TRT affect fertility?
Yes. Exogenous testosterone suppresses the body's own production of testosterone and sperm, and in some men that suppression does not fully reverse after stopping. We do not prescribe testosterone to men actively trying to conceive. If future fertility matters to you, tell us at your consultation — alternative protocols exist that raise testosterone without suppressing your own production, and they should be discussed before therapy starts rather than after.
Can I combine TRT with peptide therapy?
Often, yes. Peptides targeting recovery, sleep quality and body composition are commonly run alongside testosterone therapy, and the two address different mechanisms. Whether stacking them is worthwhile depends on your goals and your labs — we will tell you plainly if we think one alone would do the job. Anything combined is prescribed and monitored together, not run as separate uncoordinated protocols.
