AmpUp Wellness services are provided under the supervision of David Patterson, APRN, a Board-Certified Adult Care Nurse Practitioner.

3. Booking & Appointments

Appointments are subject to availability. We reserve the right to refuse service to anyone for clinical or safety reasons. A consultation may be required before certain treatments can be provided.

4. Cancellation Policy

Please provide a minimum of 24 hours notice to cancel or reschedule an appointment. Late cancellations or no-shows may result in a cancellation fee. Please contact us as early as possible if you need to make changes.

5. Payment

Payment is due at time of service. We accept major credit cards and other payment methods as displayed at our clinic. Prices are subject to change without notice. Prepaid services and packages are non-refundable once services have been rendered.

6. SMS Messaging Terms

By opting in to receive SMS messages from AmpUp Wellness, you agree to the following:

AmpUp Wellness is not liable for adverse outcomes resulting from incomplete or inaccurate health information provided by the patient. Certain treatments may not be appropriate for all individuals. Our provider will assess your eligibility during consultation.

8. Limitation of Liability

AmpUp Wellness is not liable for any indirect, incidental, or consequential damages arising from your use of our website or services. Our total liability to you for any claim shall not exceed the amount paid for the service in question. Nothing in these Terms limits our liability for gross negligence or willful misconduct.
